Case Management

CVAA Case Managers can help you get the help you need.

Champlain Valley Agency on Aging Case Managers are accomplished at coordinating a variety of services that allow elders to remain in their homes and in their communities, while helping them to maintain their independence as much as possible. Our Case Managers are extensively trained in all services and public benefits available to elders.

Case Managers will visit with elders in their homes and:

  • Assess the strengths and needs of the senior
  • Discuss options available to meet these needs
  • Put in place the services the elder is eligible for and desires
  • Coordinate and monitor those services for quality and to the senior's satisfaction

Referrals come from a variety of sources:

  • Seniors themselves
  • Doctors, nurses & hospital discharge planners
  • Family members & neighbors
  • Police, Adult Protective Services, etc.

The Senior HelpLine screens the information and, if possible, assists the caller with issues of importance. If it appears that further assistance is required, a referral is given to a Case Manager.

Case Managers work closely with a variety of community and state organizations and serve as a case manager for the Vermont Medicaid Waiver Program.
